introduction
- patients with acute onset generalised aches and pains should be initially worked up to exclude underlying sepsis / septicaemia, even if they are afebrile (consider WCC, CRP, blood cultures, etc) or viraemia such as influenza
- patients who have spent prolonged periods on hard surfaces should have a creatinine kinase (CK) to exclude rhabdomyolysis
- patients on statins may have statin-induced myopathy which typically presents as pain in thighs - these patients have a raised creatinine kinase (CK) and should have their statin medication ceased.
differential diagnoses of acute muscle aches and pains
- post-exertional:
- transient lactic acid build up in muscles
- muscle strain
- compartment syndrome
- regional ischaemia:
- compartment syndrome
