• active against a variety of Gram -ves & +ves by inhib. of DNA gyrase
• activity includes Enterobacteriacaea, Pseudomonas, Neisseria, and at higher levels, Staph, Legionella, Chlamydia & some mycobacteria with anaerobes being less susceptible.
• not for pneumococcal infection

• usual treatment duration: 7-14 days (treat for greater than or equal to 2 days after patient asymptomatic).
• uncomplicated urinary tract infections (UTIs) / cystitis: 250 mg twice daily;
• complicated urinary tract infections (UTIs) / cystitis: 500 mg twice daily.
• bronchial, skin, skin structure infection: 500 or 750 mg twice daily.
• gonorrhoeal urethritis: single 250 mg dose.
• infectious diarrhoea: 500 mg twice daily for 5 days.
• chronic bacterial prostatitis: 250-500 mg twice daily for 14-28 days.
• bone, joint infection: 750 mg twice daily, may treat for greater than or equal to 4-6 wks.
• inhalational anthrax. Adults: 500 mg twice daily; children: 15 mg/kg/dose (max 500 mg/dose) twice daily as soon as possible after suspected or confirmed exposure; treat for 60 days.
• Gram +ve infection: may admin 500 mg every 8 hrs.
• meningococcal chemoprophylaxis (adult and child 12 years or more): 500 mg orally, as a single dose

• ClCr 31-60 mL/min/1.73 m2: max 1000 mg/day;
• ClCr less than or equal to 30 mL/min/1.73 m2: max 500 mg/day;

• admin by IV infusion into large vein over 60 min
• transfer to oral therapy as soon as practicable.
• treat for greater than or equal to 2 days after symptom resolution; usually 7-14 days
• adults: 200-300 mg every 12 hrs
